New referral form for ErinoakKids services

ErinoakKids has developed a new referral form for access to our services. This new form is available as of October 1, 2015 and should exclusively be used when referring clients to ErinoakKids services. The new form is available on our website at erinoakkids.ca/referrals. The old referral form will no longer be accepted after December 31, 2015. 
Along with the new referral form, there are two additional changes we’d like to share with you.
  1. We will now be serving children with torticollis – a condition where a child’s head persistently turns to one side. If you suspect a child may have this diagnosis, please refer them to ErinoakKids for assessment and treatment.

  2. As of October 1, 2015, a physician’s referral for Occupational Therapy and Physiotherapy services at ErinoakKids is no longer required. There are still referral criteria that must be met to receive these services, which are described on the Occupational Therapy and Physiotherapy sections of our website.
