ASD Diagnostic Hub

 Referral Criteria: Children at risk for ASD may exhibit the following signs and require further professional evaluation:

  • Social withdrawal
  • Limited play skills
  • Lack of social and emotional reciprocity
  • Delayed language development
  • Persistent repetition of words or phrases (echolalia)
  • Poor eye contact
  • Difficulties with transitions
  • Restricted interests
  • Repetitive motor behaviors (flapping, rocking, spinning, etc.)
  • Unusual reactions to sounds, smells, tastes, textures, lights and/or colors

Who Can Refer: Physician or Nurse Practitioner (Please include OHIP billing information and all relevant supporting documents)

Who is part of the team:  Psychologist/ Paediatrician and clinical team of Psychometrists, Occupational Therapist and Speech Language Pathologist

Referrals for Medical Clinics must originate from a Physician. Families/caregivers may submit this form, but they must upload the Physician's referral document with all required supporting materials.
