Changes to Coordinated Service Planning Model for Peel Region

In 2017, ErinoakKids became lead agency for Peel Coordinated Service Planning (CSP), and began providing intensive service coordination to families whose children/youth have multiple and/or complex needs requiring numerous specialized services. CSP was designed to provide short-term supports in response to a family’s urgent need for system wide planning with multiple service providers. Once supports were provided and family goals met, ErinoakKids worked with community partners to transition families back to their initial, referring agency for ongoing service. 

Over time, we heard from the Peel community that having multiple service coordination agencies involved was a challenge, and so began work with our CSP partners and the Ministry of Children, Community and Social Services to improve the service delivery model by making it more streamlined for families. 
 
To this end, as of April 1, 2022, an integrated community based model was implemented by sub-contracting the delivery of CSP services to the following partner agencies: Brampton Caledon Community Living, Community Living Mississauga, Kerry’s Place Autism Services, and Child Development Resource Connection Peel. 

ErinoakKids will continue to offer CSP services to children and youth living in Peel who are medically fragile, technologically dependent (MFTD) or have (suspected or diagnosed) Fetal Alcohol Syndrome Disorder. The change to an integrated community based service delivery model better supports families with continuity of care throughout their service planning journey.

ErinoakKids is grateful to our partner agencies for their willingness to work collaboratively to adapt the way in which CSP services are delivered across the communities of Peel.
