Families, schools, physicians and community partners are now able to access our School Based Rehabilitation Services (SBRS) and General Referral form through www.erinoakkids.ca/referrals

This online form replaces the PDF/paper forms used in the past. Moving to digital referrals will streamline the referral process and help ensure that we receive all of the information required as quickly as possible so that we can start providing services to the children/youth being referred. 

Families without internet access may still complete a referral by phone with Intake. Preschool Speech and Language (PSL) referrals will continue to be conducted through the Communication Check Up link.
